Mental Health Care Power of Attorney
Similar to a health care power of attorney, this document appoints agents to make mental health care-related decisions if you become unable to make your own decisions due to mental or physical illness, injury, disability or incapacity. The agent may be authorized to
- Handle your medical records
- Consent to the administration of medications
- Admit you to a structured treatment setting with 24 hour-a-day supervision in a level one behavioral health facility.
